Clinical Lead, Knee & Hip Pathway

CareChain triages patients off the orthopaedic specialist wait-list before they ever reach a surgeon. As Clinical Lead for the knee and hip pathway, you own the clinical logic that decides who needs an orthopaedic consult and who is better served by a structured care plan delivered closer to home.

You will work alongside Allied Health Providers (AHPs), data scientists, and our anchor partner at the McGill University Health Centre (MUHC) to keep the pathway safe, evidence-based, and measurable. This is a clinical governance role with real authority over what we ship, not an advisory seat.

What you'll do

  • Own the knee and hip triage pathway end to end, from intake questions through to the rules that route 85% of patients off the specialist wait-list and the 15% who need a surgeon to a pre-screened consult.
  • Maintain the evidence base behind each decision point, grounding thresholds in validated instruments such as STarT-Back and the Oswestry Disability Index (ODI).
  • Review flagged cases and audit triage decisions monthly, so our published outcomes hold up to clinical and academic scrutiny.
  • Partner with AHPs across 40+ clinics to keep care plans consistent and to close the loop on patients who deteriorate or need escalation.
  • Work with our decision-support team to translate pathway logic into AI-supported decision support that clinicians trust and can override.

What we're looking for

  • A licensed clinician (physician, physiotherapist, or equivalent) with 7+ years in musculoskeletal or orthopaedic care.
  • Direct experience with knee and hip presentations and the orthopaedic referral threshold, ideally inside a Canadian outpatient setting.
  • Comfort reading and applying clinical evidence, including the literature on triage models such as Mutsekwa 2019 and Liddy 2020.
  • A track record of clinical governance, audit, or quality improvement where you owned outcomes rather than recommendations.
  • Bonus: experience standing up a new pathway or working with AI-supported decision support in a regulated setting.
